Poly Pharmacy-Induced Long-QT Syndrome and Torsades de Pointes: A Case Report

چکیده
Polypharmacy-induced QT prolongation is a possible hazardous adverse effect of several medication combinations. When QT prolongation leads to torsade de pointes, life-threatening or mortal outcomes can result. A 45-year-old man presented to the emergency department due to the use of methadone in the field with ventricular tachycardia and after successful resuscitation after cardiac arrest, under ICU care, has been exacerbated torsade de point due to polypharmacy with high-dose methadone, the patient had multiple risk factors for prolonged QT syndrome including methadone therapy, multiple drug therapy leading to potential drug interactions, electrolyte disturbances such as hypomagnesaemia. Clinical pharmacy must be aware of multidrug interactions potentiating QT prolongation and leading to torsade de pointes. In this article has emphasized the importance of considering the cumulative effects of several drugs used in the ICU and their timely treatment and possible avoidance of poly

Torsades de pointes: Should ECG be taken from all patients before surgery? A case report
Torsades de pointes is a rare but potentially lethal arrhythmia which mainly occurs in the setting of a prolonged QT interval. ECG is a reliable tool to detect such abnormalities, routinely taken from all patients over 40 who undergo surgery. We describe the case of a 35-year-old woman with torsades de points arrhythmia after hysterectomy surgery. Most likely, our patients had long QT syndro...
متن کاملTorsades de pointes during laparoscopic adrenalectomy of a pheochromocytoma: a case report
INTRODUCTION Torsades de pointes is a rare but potentially lethal arrhythmia. The amount of literature available on Torsades de pointes occurring in patients with pheochromocytoma is limited, and we found no literature describing this dysrhythmia in a patient with pheochromocytoma under anesthesia. CASE PRESENTATION We describe the case of a 42-year-old Caucasian woman without QT prolongation...
متن کاملAcquired Long QT Syndrome and Torsade de Pointes Associated with HIV Infection
Here, we report the case of an HIV infected patient that was treated for pneumonia with a macrolid antibiotic. The patient experienced a prolongation of the already pathologic QTc interval resulting in repeated torsades de pointes necessitating CPR and implantation of an AICD. This case exemplifies that torsades de pointes due to acquired long QT syndrome is a serious and potentially fatal comp...
متن کاملCiprofloxacin induced acquired long QT syndrome in a patient under class III antiarrhythmic therapy.
We report one case of cardiac arrest related to ciprofloxacin administration. One female patient (aged 70 years old) developed a marked QTc prolongation (QTc = 0.62 s) within 24 hours of ciprofloxacin administration, with documented torsades de pointes and recurrent syncope that required defibrillation. The patient was under amiodarone and sotalol therapy for atrial fibrillation, with no obviou...
متن کاملProbucol-induced QT prolongation and torsades de pointes.
Probucol administration of 4 weeks produced torsades de pointes associated with exacerbated QT interval prolongation in a 36-year-old woman with Romano-Ward syndrome. With discontinuance of probucol, the QT interval corrected for rate shortened from 620 msec to 500 msec and ventricular ectopic beats disappeared completely. Although probucol is known to prolong the QT interval, associated ventri...
